WAIVER AGREEMENT

I recognize and acknowledge that when participating in any Fort Collins Pickleball Club Pickleball events and activities that there are certain risks of physical injury or infection to participants. I agree to assume all risks, and accept personal responsibility for any injury to myself or a minor if I am registering them for an event (including, but not limited to, personal injury, disability, and death). This includes any illness, damage, loss, claim, liability or expense, of any kind or nature, that I may suffer arising out of or in connection with becoming exposed to or infected by COVID-19 while participating in any event or program offered or advertised or sponsored by the Fort Collins Pickleball Club. I hereby release, covenant not to sue, and forever discharge the Fort Collins Pickleball Club, its officers, directors, employees, agents, subcontractors and representatives, of and from all liabilities. I waive and relinquish all claims that I or my insurer may have against the officers, directors, employees, agents, subcontractors and representatives from any and all claims from injuries, damages or loss which I may have or which may accrue to me on account of my participation in any event or program offered or advertised or sponsored by the Fort Collins Pickleball Club.
